Report of Birth and Obtain Non-Japanese Registration Certificate for Your Child

Within 14 days after a birth of child, father or mother must make the report of birth with the birth certificate to municipal office. Bring the maternity handbook at that time.
Apply national health insurance if you want.
If he or she has a nationality other than Japan, Non-Japanese Registration Certificate must be applied with an acceptance of birth certificate (it will be issued at the section which you reported the certificate), or maternity handbook within 60 days after the date of birth. Either father or mother who live with the child must do the application.
Also for a nationality of the newborn child, he or she can obtain a Japanese nationality if either child's father or mother is Japanese. If one of the parents is non-Japanese the child can also obtain the parent's native nationality. (A double nationality)
For such case, please report the reserve of the nationality with his/her report of birth. When child has a double nationality, the child must chose either one by the time he or she becomes 22 years old. Ask municipal office citizens' affairs section (Tel: 20-1525) for details.
